ITS 340:5-5-3(e)
Before closing a case on reason of Unable to Locate, be sure the following steps have been followed:
- Make the initial visit face to face or on Service Cases, a phone call if assigned as such.
- If it was a face to face attempt and there is no answer at their door call their phone number and try to make contact with them.
- If there is no answer, call the reporter and inquire about the AV’s whereabouts.
- You can also call collaterals and inquire about the AV’s whereabouts.
- If you are at their home in person, and there is no answer leave your card/Q & A Booklet with contact number in their door.
- If the referral is a service case and phone contact wasn’t made with the AV, a face to face visit must be attempted within 5 days. Go to step 2.
- A second visit should be attempted within 5 business days. Be sure the second attempt is at a different time of the day.
- If you are unable to make contact with the AV after the second attempt, send a letter to the AV requesting a visit and give them seven calendar days to respond.
- If there is no response to the letter, the case is staffed with supervisor for case to be closed as unable to locate.
- When the APS specialist is still unable to locate the AV, the APS specialist notifies the reporter again.
