Medicaid Involved

When the need for Medicaid services is identified in a case, the specialist will refer client to Medicaid involved services within three days of recognizing the need for the service. These services may include:

ADvantage Waiver Program

The Home and Community Based ADvantage Waiver program provides alternatives to placement in a nursing facility for eligible adults. The program helps older adults and adults with disabilities stay independent and close to family and friends. ADvantage promotes individual choice and self-direction.

ADvantage Assisted Living

Assisted Living is a homelike, non-institutional setting offering services for those who may need daily assistance but not continuous nursing care. Assisted Living facilities offer services such as: daily meals, housekeeping, laundry, help with taking medications, personal care, intermittent or unscheduled nursing care, assistance with transfer and ambulation and social, recreational and exercise opportunities. Any ADvantage Member can request the Assisted Living service option.

Adult Day Services (state funded)

Adult day health services meet the physical, social and emotional needs of older adults and those adults with disabilities. DHS contracts with adult day health centers across the state to support the health, nutritional, social and daily living needs of adults in a professionally staffed, group setting. Services provided include health monitoring and medication administration; socialization, leisure, recreational and educational activities; assistance with personal care; nutritious lunches and snacks; transportation and counseling.

State Plan Personal Care (state funded)

State Plan Personal Care is a Medicaid service that helps individuals, from children through aging adults, with daily living activities at home.   A personal care attendant helps the individual with things like bathing, grooming, getting in and out of bed, toileting, meals, eating, laundry tasks, shopping/errand tasks and light housekeeping. Skilled nursing services include assessment of need and development and oversight of a care plan.
