Example of Appropriate Findings Narrative
Substantiation of abandonment:
AV’s daughter has been taking care of her for the last four years. She left her at the ER two day ago and has not been back to check on her. No one can get in touch with her. Her phone has been disconnected and there is no one in the home. It appears she has moved. She did not leave a forwarding address.
Unsubstantiated allegation of abandonment:
AV’s son has stopped nutrition to his father. It appears to his neighbors that his son is starving him. It is discovered that AV has completed all of his own Hospice paperwork, and he has chosen to stop nutrition himself.
