Home Inventory

  1. Specialist staffs with supervisor to determine a day to complete the inventory. Specialist will need to arrange to have a third-party witness that will be present the entire time of the inventory. Please note if the third party must leave at any point of the inventory, the inventory will need to stop with the house locked until the third party can return to be present. If inventory is stopped, please have third party and all present sign the inventory and note where inventory was ended and where it will resume in the home. This way if a different third party is used to complete the remaining inventory, the initial inventory is documented. Inventory should be completed in pairs so that no one is left alone in the process. This is for staff’s protection against any lost or missing items.
  2. Inventory needs to be completed by photo, written documentation or both as assigned by the supervisor or designee. If photo inventory is assigned, there needs to be a written statement signed by all present, including third-party witness.
  3. The inventory statement should have a date, client’s name, location of inventory, every person present will document their name and title/relationship to client.
  4. Staff should take all efforts to adequately secure client’s personal belongings in the client’s home. It should be rare instances that items are removed from the client’s house to be stored in the county office, with the exception of keys. Items should be stored in the office only on a temporary basis. If after staffing with District PFR/DD, it is determined that there is a need for items to be removed from the home, the items should be taken to the designated location to be secured, the items should be included on the original inventory and also documented on a separate inventory sheet to document what property was taken with clear description of item and photo of item and where it is stored. At no time should client’s property be stored in specialist’s home/car.
  5. If property is removed from the home and taken to the nursing home to be with client, the items should be included on the original inventory and also documented on a separate inventory sheet to document what property was taken with clear description of item and photo of item. This inventory sheet should also include the nursing home staff that received the item along with title, signature and date.
  6. If home is entered after the inventory is completed, staff must have third party witness, list items that are removed from the home and complete step 4 or 5 depending on where the removed items are taken.
  7. Once completed, photos and inventory sheets should be stored under Files in AVA.
