72 Hour Guardianship Tasks

1. Arrange transportation to placement.
2. Secure the home and client’s personal property.
3. Arrange for animal care if needed.
4. Notify financial institutions to freeze or place alerts on accounts.
5. Obtain a printout of the bank account to verify account balances.
6. Notify law enforcement to watch the home.
7. Initiate the + Guardianship button in AVA and complete NH Placement Protocol (only if petitioning for guardianship of person) if nursing home placement needed and placement request in AVA.
8. Notify Veteran’s Affair if adult is a Veteran.
9. Notify Tribal Court if adult is a tribal member.

Note: Nursing Home Placement protocols for identified placement should be completed prior to petitioning for guardianship. If petition is being sought for AV to be placed in the hospital for medical treatment, the placement protocol is not needed prior to petitioning. Nursing Home Placement Protocol should be completed prior to discharge from the hospital in that situation.  Nursing Home Placement Protocol is NOT necessary if guardianship is for Estate only.

If your client is already in a facility when guardianship is taken the Nursing Home Placement Protocol are still required for that facility.
