- Arrange for lawn care if needed. (this step is only done if estate only or person and estate is included in the guardianship order)
- Winterize home if needed. (this step is only done if estate only or person and estate is included in the guardianship order)
- If client resides in rental property staff to determine how to dispose of personal property. (this step is only done if estate only or person and estate is included in the guardianship order)
- If personal property needs to be sold, look for titles, keys and staff with PFR/DD or designee for possible sale. This will require a court order to sell, dispose, or discard as all are options. (this step is only done if estate only or person and estate is included in the guardianship order)
- Follow up on Medicaid application monthly until approved, entering a Service Plan Monitoring Note each month regarding the status.
