The process for requesting a placement approval is as follows :
- Review Oklahoma State Department of Health (Surveys of Long-Term Care Facilities) for inspections and deficiencies. Place a copy in your files In AVA. You will go to the Search at the top of the page to start the search for the facility.
- Email District PFR to request a review of the LTC investigation at the facility being considered for placement.
- Contact the Ombudsman for a facility review in which the specialist asks if the ombudsman has any knowledge of problems at the facility or concerns with placement at the facility. This should be documented in the comments section of the placement request.
- Once a potential placement is located, the assigned specialist will submit the placement.
Example
Requesting placement at ABC Nursing Home. OSDH shows last inspection on 2/21/23 with deficiencies in medication dispensing that constitutes actual harm to resident. Number of deficiencies was 3. Contacted Nancy Drew Ombudsman who reports the facility has struggled with patient care but are improving.
NOTE: Nursing Home Placement Protocol is only needed if guardianship is for Person or Person and Estate. It is not necessary if petition is only for guardianship of Estate.
