While the RED Team or supervisor conducts most of the diligent searches during screening and adds the information as a “Screening Note,” field staff are responsible for researching criminal history and Harmony then adding the information as a “Diligent Search” note.
In addition, field staff will still complete a thorough diligent search. If there is additional information found that was not found during screening, include it in the “Diligent Search” note. If there is no additional information to add, the “Diligent Search” note will consist of criminal history and Harmony information only. There is no need to copy and paste the information found during screening into the “Diligent Search” note, as the information already exists in the “Screening Note.”

Diligent Search Note Template
After the diligent search is conducted, there is a standardized note template to use. To add the diligent search note in AVA, please click the New Note button, change the Type from General to Diligent Search, and then copy and paste the following template:

Here is an example of a sufficient diligent search note:
OSCN/DOC: OSCN shows CM- 2015- 5689 for DUI for client and civil cases. OSCN shows CF-2023-5678 pending for possession of a controlled substance and CF-2005-894 for assault and battery, possession of controlled substance with intent to distribute sentenced to DOC 5 years for listed AP. DOC shows time served and released date of 2009.
Harmony: Reviewed and client has Dillan Home Health. Case manager added as collateral.
