- Change of Placement: All out of home placement, including any change of placement of a vulnerable adult under APS guardianship, are reported to and subject to approval of the Court.
- Once initial placement is approved in a guardianship, if it is determined that a change of placement is needed, Steps 1-3 on Nursing Home Placement Protocol should be completed.
- Once placement is located, the Court of guardianship should be notified.
