The account is established using the name of the APS specialist, and another APS employee, preferably the APS supervisor or Program Field Representative. The signature page for the account must be signed by the two APS employees named on the account. A copy of the signed signature page must be saved as Guardianship Account Signature Page to the case in files.
Financial Institutions that require the APS specialists’ social security numbers are giving the OKDHS federal employee identification (FEI) number 73-6017987. In no instance does the APS specialist provide his or her personal Social Security number for the vulnerable adult’s financial account.
Provide the bank with a copy of your APS badge, it is approved by APS legal for the specialist to provide a copy of their driver’s license to establish the account if needed.
Payment and documentation of the transactions for the Guardianship account are saved to the asset management tab in the case.
