Capacity Statement Examples

  • Client does not appear to have capacity to consent as she is unable to describe her health problems.  She does not recognize the risks and consequences of not taking her diabetic medication or of her blood sugar running high.  Her awareness of the resources available to assist her is limited.
  • Client appears to have capacity to consent.  She is able to describe her difficulty ambulating and the risk and consequences of falling.  She also knows how to access emergency medical assistance if needed.
